A Bismark is in or behind smoke and hasn't fired the main guns.  A red dd is spotted 8 km out by another ship. Bismark goes CNTRL + LMB (secondaries main focus) . Can the dd spot him at that point?

I don't think so. I don't have Bismarck myself but when my secondaries go off in Leander within 4.6 km I don't get spotted.

The penalty only applies to main guns. Secondaries, AA, and torpedoes will not get you detected.

I have done this in both the Tirpitz and Bismarck, sat in smoke and just rained secondary hell upon an enemy.
