Any ship with a boiler could potentially make smoke, some had smoke canisters.

doubtful, since WG is changing BB detection when firing from inside of smoke.

I think that if you have 3 or more fires on you, you should get some kind of concealment bonus from all that smoke, but that's just me.

When ships are on fire they should be creating a big 'ol smoke cloud. I'm a retired Fireman and I never saw a building/vehicle/tree on fire that didn't generate a lot of smoke.

Technically smoke is produced by evaporating oil based product. Destroyers and cruisers sometimes had dedicated equipment on deck for screening purposes. However, any ship that introduced fuel oil to a hot smoke stack could produce a smoke screen. 

Also, ships before 1930's (approximately?) did not use smokeless powder if their guns and ships battle lines should produce obscuring smoke just by sustaining main battery fire at low speeds. Of course they couldn't see out of it either.
